1. Preventing CAUTI's

1.1. Always do Foley Care each shift with CHG wipes.

1.2. hand off should include date placed and reason for keeping Foley and plan to remove it.

2.3.1. AIDET stands for Acknowledge the patient by name. I= introduce yourself and tell something about you to start the connection and give the patient confidence. D=duration. Tell the patient how long whatever it is you are doing will take. E is explanation. Explain each thing before you do it, to help alleviate anxiety. T is for Thank you. A simple thank you can go a long way.

2.4. Use ITRACE to go over everything touching the patient

2.4.1. Definition: ITRACE: I is Illuminate (to look at everything that is connected to your patient. T to Touch & Trace (to touch and go along each tubing, line touching your patient and investigate and check. R is Review is to think about what it is for and if it is doing what it is intended to do correctly. A is for Act. If there is a problem or a mistake, fix it. C is for Correct or Clarify. Explain anything needed for patient or family. E is for Expect. Always expect to use this process when doing hand-off.
