1. Express concern for the injured party and make an initial assessment of the injury

1.1. Determine if the injury requires first aid or immediate medical attention

1.2. If first aid is needed provide the necessary aid

1.3. If immediate medical attention is required contact the local authorities - calling 911

2. Complete the Proper Reporting Form

2.1. Forms should be completed iwth in 24 hours of the incident

2.2. All forms should be completed by the manager on duty

2.2.1. Forms should never be given to a guest or an employee to complete

2.2.2. Copies of completed forms should not be distributed to any injured party by the location

2.2.3. Employees should not make any statements regarding prior injuries or the company's liability

3. Submit Completed Forms

3.1. Fax forms to 888-208-8960

3.2. Email forms to [email protected]

3.3. Email any pictures of the area involved in a Slip/Fall claim to [email protected]

3.4. Send any witness statements or comments made by those involved to [email protected]

3.5. Any products such as furntiure that causes someone to become injured should be kept in a secure place

4. After the claim has been reported

4.1. Managers should expect to recieve a call from the assigned adjuster

4.2. Send any further correspondances received to [email protected]

4.3. Should notify Heather Hinsdale right away if the injured person contacts the location

4.4. Follow up with injured employees each week

4.4.1. Work with Heather Hinsdale on returning employees to work with light duty restrictions

5. All Management Staff

5.1. Contact - Heather Hinsdale

6. Managers and Staff shoud NEVER

6.1. Offer their opinion as to the cause of the accident

6.2. Never except liability or offer to pay for medical treatment

6.3. Never discuss the company's operations or prior incidents
