history taking, HPC, malaise for 3 months, gradual worsening, hx of diarrhea, chronic mild diarrhea since late teens, 3-4 bowel motion per day .. occasionally at night, sttols is loose and bulky .. pale in color .. malodorus .. difficult to flush .. no blood or mucus .. long standing flatulence, abdominal bloating, prior to bowel motion, relieved by passing stools or flatus, symptoms are aggravated by milk, slight weight lost 2 kg, monthly menstrual period .. light blood loss .. lasts for 2 days, PH, intermittent mouth ulcer since childhood, menerache at age of 15, FH, no, SH, no smoking no alcohol, no travel, born in reiyadh, normal diet .. having meets 3 times?week, her father doesn't drink milk, medication history, occupitional history, school teacher

physical examination, Vital signs, pulse is 68, BP 110/70, h: 158 .. w: 48, inspection, looks pale, abdominal examination, inspection, abdomin not distended, no tenderness no masses, auscultation, increase in bowel sounds, normal anal, chest, normal, CVS, soft systolic ejection murmur .. at the left external margin
