1.1. Video conferencing is especially beneficial fro a patient with a long term condition: for example COPD patient may not be able to travel far, video conferencing can make a positive impact for these patients.

1.2. Nursing practice can be involved as community nurses can educate the patient about video conferencing  in their own home and make the suggestion and the referral for any upcoming consultations.

1.3. The video conferencing aims to promote the health and social care integration by providing digital tools for information gathering.

1.3.1. The aim is to support patients to digitally manage their own health and well being by leading healthier lives at home and in the community setting

2.1. As stated above this will assist in the health and social care integration.

2.2. The video conferencing can be used between professionals  to communicate about a patients treatment. For example a community nurse can communicate with a specialist to devise a care plan suitable for the patient.

2.3. The patient can gain empowerment as they will be able to stay in the home setting and be educated  whilst staying at home to improve their health.
