2. Basic information

2.2. If 14 years of age or older, the individual education plan must include a plan for the transition to appropriate post secondary activities, such as work, further education, and community living.

3.1.1.1. Prepare student in advance of transition (eg. Calendar), visit a new school or location in advance, use consistent cues/routines

4. Steps in the process

4.1. 1) Select a team leader (often the person writing the IEP) and team (anyone the student would like to have help - parents, teachers, community members such as the Community Living Association, etc.)

4.1.1. 2) gather relevant documentation for student (OSR, information from relevant community organizations, etc.)

4.1.1.1. 3) Meeting (the student and team members)

4.1.1.1.1. 4) Identify goals based on student's strengths, needs and interests (eg. preparing to obtain employment)
