2.1.2.1. • Consider remediation of site or corrosion control for the system • Distribute Consumer Notification Form with new Result include High Result addendum
3. If your result is above 15 ppb lead or 1300 ppb copper
3.1. ACTIONS TO BE PERFORMED WITHIN 72 HOURS: • Distribute Consumer Notification Form with High Result Addendum ACTIONS TO BE PERFORMED WITHIN 30 DAYS: • Submit Certification of Notice to the Division • Conduct Follow up Sampling • 1 Tap • 1 Flush • 1 Meter

ACTIONS TO BE PERFORMED WITHIN 72 HOURS: • Distribute Consumer Notification Form To homeowner with High Result Addendum ACTIONS TO BE PERFORMED WITHIN 30 DAYS: • Submit Certification of Notice to the Division • Conduct Follow up Sampling for each site above 13ppm lead or 1200 ppb copper • 2 Tap (take on different days) • 1 Flush • 1 Meter
4.1.1. If both repeat tap results are below 13 ppb lead or 1200 ppb copper:
4.1.1.1. ACTIONS TO BE PERFORMED WITHIN 30 DAYS: • Distribute Consumer Notification Form with new result • Conduct Follow up Sampling for each of the sites that originally tested above 13ppb lead or 1200 ppb copper • 2 Tap (taken on different days)
4.1.1.1.1. If both repeat tap results are below 13 ppb lead or 1200 ppb copper:
4.1.1.1.2. If either of the tap results is above 13 ppb lead or 1200 ppb copper
