Complete Proposal Application.

Obtain a copy of product liability certificate of insurance.

Make a copy of your Uniform Code Council (UPC) (GSI) letter identifying your company\'s bar code labeling.

If you are a minority/women-owned business, fax copy of current minority/women-owned business certificate to (479)277-2532.

After your submission packet is completed and emailed, you will recieve an answer from a buyer. If they are interested, you will be asked to provide a sample of the product and a copy of your UPC Code letter to an address they will provide for you.

If they decide to sign you on as a supplier company, you will be contacted again to meet with a Corporate Buyer and begin the contract processes.
