1. Time/course

1.1. Progression

1.1.1. Earliest signs of pre diabetes is insulin resistance in the patient. At this stage however the pancreatic B-cell is still able to compensate by secreting insulin. This is called the pre-diabetes stage.

1.1.2. From here patients progress to the impaired glucose tolerance stage of diabetes. This is caused by hyperinsulinemia. Over time the B cells become refractory to glucose causing an insulin deficiency to develope (Fonseca, 2009).

1.1.3. Later in the diseases progression the B cell will deteriorate even further leading to an absolute insulin deficiency. This will result in the B cell becoming unresponsive to medical interventions.

1.1.4. At this point the patient requires exogenous insulin. This is the insulin-requiring stage of type 2 diabetes.

3. Diagnostics

3.1. Conventional diagnostics

3.1.1. Laboratory tests

3.1.1.1. HbA1c (glycated haemoglobin test)

3.1.1.1.1. Glycated haemoglobin test HbA1c: It indicates blood sugar levels from the past 2-3 months. Levels above 6.5% on two seperate tests is indicative of diabetes.

3.1.1.2. Random blood glucose test

3.1.1.2.1. If the A1c test is not conisisten the following tests may be used. - random blood glucose test: higher than 11.11 mmol/L is indicative of diabetes("Type 2 diabetes - Diagnosis and treatment Mayo Clinic", 2021).

3.1.1.3. Fasting blood glucose test

3.1.1.3.1. 7mmol/l or higher is considered diabetes.

3.1.1.4. Oral glucose tolerance test

3.1.1.4.1. Fasting (8 hours), 75g of glucose is then administered, blood is then collected from sample and two hours post glucose uptake. Considered diabetes if after 2 hours if blood glucose is equal or more than 200mg/dL

3.2. Osteopathic diagnostics

3.2.1. Case history

3.2.1.1. Insidious onset.

3.2.1.2. Primary complaint is fatigue.

3.2.1.3. Previous history of coronary artery disease, hypertension, claudication, polydipsia, nocturia, weight loss and fatigue.

3.2.1.4. Has the patient had a cerebrovascular accident ?

3.2.1.5. History of neuropathy or symptoms of peripheral neuropathy

3.2.1.6. Evaulate their diet and exercsise levels.

3.2.1.7. Do they have any foot ulcers ?

3.2.2. Does the patient have hyperlipedemia ? When was their last test ?

3.2.3. Physical exam

3.2.3.1. Systems examination

3.2.3.1.1. Neurological examination of lower limb.

3.2.3.1.2. Observe for skin changes (coolness,paleness, dryness, pimples and abnormal pigmentation).

3.2.3.1.3. Opthalmic examination assessing the retina, macula and optic disc.

3.2.3.1.4. Vital signs examination assessing blood pressure, temperature and weight.

3.2.3.2. Palpation

3.2.3.2.1. Assess for tissue changes such as doughy, ropey and thickened interstitial tissue.

3.2.3.2.2. Assess for viscerosomatic reflex changes at the T11-L2 area(Licciardone et al., 2007).

3.2.3.3. Foot examination

3.2.3.3.1. Assess the dorsalis pedis and posterior tibailis pulses.

3.2.3.3.2. Observe for skin changes on the foot such as ulcerations.

5. Pathophysiology

5.1. Type 2 diabtes is caused by a combination of two primary factors.

5.2. 1. Defective insuline secretion by the B-cells of the pancreas.

5.3. 2. Tissue inability to respond appropraitely to the insulin (Mukhamedzhanov & Esyrev, 2013). These two factors are caused by the following mechanisms.

5.4. Malfunctioning of the feedback loop between the secretion of insulin and it's function causes high levels of glucose in the blood.

5.5. As the disease progresses Beta cells begin to change resulting in B-cell dysfunction. This results in reduced insulin secretion which then limits the bodys ability to maintain glucose levels (Galicia-Garcia et al., 2020).

5.6. Simultaneously insulin resistance will increase glucose production in the liver, however will decrease the glucose uptake in the muscle and adipose tissue.

5.7. When both B-cell dysfunction and insulin resistance are present the patients body will become hyperglycaemic leading to the progression of type 2 diabetes.
