2. Accessing the services and programs

2.1. To access the program one needs to fill out the referral form, individuals, parents or legal guardians, physicians and other community agencies can make a referral by completing the referral form above or by calling the Centre at 519-252-7281.

2.2. Admission Criteria Students who can actively participate in... An intensive post traumatic/post operative rehabilitation program Intensive assessment and training related to complicated rehabilitation needs A protective environment that meets their educational needs.

3.1. In regards to cultural diversity and ELL there is no specific programming for them outlined in JMCC website however the site does state that: The JMCC School is dedicated to providing a child centred, personalized and precise education that provides assessment and instruction designed to meet your child's particular learning and motivational needs. 5.1. The John McGivney Children's Centre provides rehabilitative services to the Windsor and Essex County community, enriching the lives of children and youth with special needs by helping them reach their full potential

6. Services and Programs

6.1. The JMCC offers many different services and programs, it runs as its own school and as a school their goals are: to work with all stakeholders in establishing precise, measureable rehabilitative and academic goals to provide the highest quality programming to meet individual student needs to maximize student development in functional communication, literacy, numeracy, and social skills to continue to forge strong working relationships with local school boards to ensure successful transitioning into community schools to continue to foster a culture of collaboration with families and centre staff to provide continued support beyond transition to community schools to ensure success for ALL students

6.2. In regards to family services the JMCC offers family service workers: The role of the Family Service Worker is to connect new families to appropriate programs and services at the John McGivney Children's Centre and in the community. This may include: A home visit or telephone contact to gather information regarding parental and medical concerns around the child's development Accessing community resources Assistance with funding for transportation, respite care and medical supplies Service Coordination Advocacy Planning for the future
