4. 8. DATA COLLECTION SYSTEM

4.1. 1. Each healthcare facility is required to collect data based on data collection format

4.2. 2. The data should be analyzed by facility at regular interval

4.3. The facility shall take pro-active measures to improve performance or prevent recurrence of similar incident at the facility

4.4. 4. The data should be submitted to Technical Coordinators of Patient Safety Council, Quality in Medical Care Section, MOH, once a year only

4.5. 5. The Technical Coordinators of Patient Safety Council will analyze data & present it to Patient Safety Council. Data will be used to plan & prioritize action to further improve patient safety at national level
