Langs Door Mind Map: Langs

1. Credo: At Langs, how we work is who we are: our expertise in neighbourhoods and the relationships we build are central to the success of the work we do. We offer care and support with respect and compassion. We include our patients and participants among our valued community partners. We work with others who share our vision to build healthy and engaged communities, where individuals are valued and supported. We know that health, wellness and access to care are affected by many factors, including income, housing, employment, education, social support, food security, gender and the environment. Langs staff and volunteers understand that this complex picture is particularly challenging for members of our community who experience multiple barriers to access.

2. Community Health Centre: A Community Health Centre (CHC) is a non-profit, publicly funded organization that provides primary health care from a team of doctors, nurse practitioners, registered nurses, registered dietitians, social workers and community health workers. The multidisciplinary team ensures the patient gets the right care, at the right time, delivered by the most appropriate provider. The focus of a CHC is on health promotion, illness prevention, and community development.

3. Langs is committed to ensuring all members of our community have equal access to the services we provide. We are committed to meeting the needs of people with disabilities in a timely manner, and will do so by preventing and removing barriers to accessibility and meeting accessibility requirements under the Accessibility for Ontarians with Disabilities Act (AODA).

4. Each day, providers have a combination of same day and pre-booked appointments. Patients are asked to call for an appointment. Appointments are booked based on need.

5. Vision: Changed lives, healthy communities.

6. Mission: Every person in our neighbourhoods will have a place to call home for health, wellness and community support.

7. A new multi-purpose Wellness Centre, bringing many community partners under one roof. The centre houses Langs as well as other community programs such as Family and Children's Services, counselling services, early childhood programs, youth and teen programs and a vocational resource centre. A community health centre and a large pharmacy are located here as well as a Seniors Centre.

8. Langs has a proven track record of being a community hub model. The organization is co-located with 25+ community partners to provide social, health, recreational, educational and vocational services on site. Locating all these services under one roof ensures that residents have access to a wide spectrum of programs and services in their local community.

9. Community Partners Include: Lutherwood, Carizon, Front Door, CMHA, Healthy Smiles Ontario, Deaf Blind Ontario Services, Community Support Connections among others
