3. Transition Curriculum

3.1. It is the list of competencies and skills arranged in complexity to train the LSENs with the appropriate skills related to their desired exit points.

3.2. It serves as the SPED teacher’s guide for crafting the instructional methods, strategies, and accommodations for a successful transition program.

4. Transition

4.1. Meaning

4.1.2.1.1. refers to the movement of a child from elementary to junior high school; junior high school to senior high school; then senior high school to undergraduate school; and undergraduate school to graduate school.

5. Individualized Transition Plan

5.1. where the LSEN is being included in the brainstorming to identify his strengths, interests, wants and dreams.

5.2. Just like the IEP, the ITP also consists of measurable transition goals as preparation for LSEN’s wants to do and what to achieve after school.

5.3. It is a part of Individual Education Plan or IEP where parents, SPED teachers, regular teachers (if the child is in inclusion), related professionals and the child meet to map long-term and short-term outcome goals and to plan the desired activities and services as preparation for an adult life.
